Nice, clean hotel on western side of Dubrovnik, 20-30 minutes from the airport, with a great balcony area to watch the sunset. Staff was responsive to a 4 AM wake up knock on the door and hailing a cab at that early hour.
Hotel Adria is situated on a hill top with stunning views of Dubrovnik. My room had a balcony to enjoy the view in the morning.
The room was spacious and spotless and the staff were lovely and helpful.

I think the hotel specifies 'max. 2 adults' for the double room. And if their Reservations staff have already said it's not possible to accommodate a third adult, I doubt you can pay extras on check-in. See if you can book a single room for your brother before you go, it'd be more comfortable for him and breakfast (they have an excellent spread) is included, their rates are very reasonable, so no point squeezing 3 in a room! Do ask for one with a view, though, our twin room had a beautiful view of the bay. I remember reception was always very busy and they may not want to entertain last-minute requests so settle everything before you go.
