We purchased from the ticketmachupicchu website. Our sales officer / ticket agent was a lady named "Gina". Yes, we had to provide copies of our passports. No, we didn't encounter any problems.

I bought the tickets via an agent then got my own guide when I was there, you can get the tickets on the site but it's a bit complicated was easier to get someone else to do it. The hostel I was in arranged the guide.
